Sunrise Manor Nursing & Rehabilitation Center provides exceptional short-term rehabilitation and long-term skilled nursing care in a warm, patient-focused environment. BAY SHORE, N.Y. - Washingtoner -- Sunrise Manor Nursing & Rehabilitation Center, an intimate 84-bed skilled nursing and post-acute care facility, today announced new executive leadership alongside a series of facility renovations that reaffirm its commitment to providing high-quality rehabilitation and long-term care services for the community.

The leadership transition comes at a pivotal time as Sunrise Manor enhances its programs and environment to better serve residents, families, and hospital partners. With a strong focus on short-term rehabilitation, post-hospital recovery, and skilled nursing services, the facility is positioning itself as a trusted partner for patients transitioning from hospitals to home or long-term care. The facility is still headed by Jared Hollweg the exceptional administrator who has lead this home through this transition.

"Sunrise Manor has always been a place where residents feel seen, supported, and cared for as individuals," said Ben Steinman, Vice President of Business Development at Sunrise Manor. "With our renovations and renewed vision, we are creating a more modern and comfortable environment while strengthening our ability to work with hospitals, physicians, and care coordinators to ensure smooth, successful discharges and recoveries."

So far, renovations have been completed on one-eighth of the resident rooms, with additional plans underway to upgrade the therapy gym, common areas, and lobby. These enhancements are designed to improve both the resident and family experience while supporting clinical excellence in rehabilitation and long-term care.

Sunrise Manor offers a full continuum of services, including physical, occupational, and speech therapy; 24-hour skilled nursing; complex medical management; and personalized care programs. The newly added targeted therapy program allows for additional therapy minutes for residents that qualify. The smaller size of the facility allows for a hands-on approach where staff, residents, and families form strong, lasting connections.
